Homeless Persons' Health Project

Our Mission: The Homeless Persons Health Project provides comprehensive health care and housing in an effort to eliminate homelessness in our community.

The Homeless Persons Health Project (HPHP) is one of four health clinics within the Health Services Agency of the County of Santa Cruz. They are located at 115-A Coral Street in Santa Cruz, HPHP provides a number of services to people experiencing homelessness and low-income populations of Santa Cruz County, including:

  • Primary care
  • Integrated Behavioral Health (therapy and psychiatry)
  • Medication Assisted Treatment
  • Acupuncture
  • Wound care
  • Benefits Advocacy

Our professional team approach to care and homeless issues emphasizes our commitment to the community.We offer respectful care and treatment, health benefit advocacy, payee services, permanent supportive housing programs and more.

If you need healthcare, information on local food and shelter programs, referrals, or related topics, stop by to see one of our public health nurses.  If you do not have insurance, we will still see you, and will even help you apply for Medi-Cal and other benefits.  Walk-ins are welcomed.  

Outreach Information & Schedule

Outreach sites and times are subject to change based on community needs.

If there is an event where you feel it would be helpful to have an HPHP representative present to provide outreach services, we are open to discussing this with you.
